Hosanna’s desire is for everyone to be able to impact people within the
four rings, while utilizing the spiritual gifts and talents that God has given them. For that reason, we offer a variety of opportunities and locations to serve.

If you have been accepted onto an Impact Team, you are about to experience a life-changing event! In preparation for your trip, your team leader will arrange for several training sessions. At your first session, you will receive a participant folder. Below are copies of the forms (also found in your folder) that will need to be completed prior to departure. Questions? Contact your team leader.
Considering Applying for a Team?
Need a new or updated passport? .pdf document
All international trips require a passport. Prior to departure, it must have more than six months before it expires. You should apply for a passport as soon as you have been accepted onto a team. Passports are currently taking 16-20 weeks for processing. Please consider expediting your application if you need it sooner than 20 weeks.
Reference Form .pdf document
All participants who have not been on a Hosanna! Impact team before must have 3 people complete a reference form. This form should be sent directly to Sheri Richards at Hosanna!
Need a Travel Clinic?
The ideal time for a consultation is six to eight weeks before your trip. The Park Nicollet Travel Clinic in St. Louis Park is the largest and most used clinic in the area, although others are available. Your regular doctor is the best resource.
Park Nicollet travel experts are available weekdays 8 a.m. to 5. p.m. Some early evening appointments are available. Call 952-993-3131 to schedule an appointment. Some travel services are covered by insurance. Call your insurance company before your appointment to find out if services are covered. Click here for directions.
2008 International Medical Insurance .pdf document
Hosanna! provides international health insurance for all out-of-country trips as most traditional health insurance does not cover care outside of the United States. The cost for this insurance is included in the trip fees. Your insurance policy information will be provided shortly before your departure. This nine-page document is a detailed description of the coverage provided for you. This is not a trip-cancellation coverage.
Forms to Bring to Panorama Training
Team members should bring these forms to their training day. If you have trouble opening these files, copies of the forms will be available at the event. Please come prepared to provide:
All contact information (home/cell/work phone and email) for 2 emergency contacts while you are overseas.
Addresses from past 7 years, driver's license & social security numbers for background check (ONLY IF driving a vehicle OR being housed with a minor)
Required Forms .pdf document
Team Covenant
Publicity Release
Emergency Contacts
Background Check .pdf document
This is only required for individuals over 18 who will be driving on the trip, OR if housed with a minor who is not your own child or immediate family member.
Panorama Training
Panorama Training is a required event for all team members, even those who have completed it in the past. This course is the foundation for our Impact team ministry, and is an important event for team interaction. This is a full-day of training in the inner city with outreach events connected with it.
Date of the Next Panorama Training: Jan 31 or Feb 7 (TBD)
What if I cannot attend the training?
If you are unable to attend, you will be required to attend a future training prior to departure. If this is not an option, listen to the Panorama teaching, and turn in a complete review sheet. If you have trouble accessing the training online (which is available as a download or streaming audio file), there are CD's that you can borrow and return. Complete the required paperwork (3-4 items) listed above.
Post-Trip Form
Evaluation .pdf document
This should be completed upon return and sent to Sheri.
